What Is the Medicare Levy?
The Medicare Levy is a 2% tax on taxable income that funds Australia’s public healthcare system (Medicare). It applies to all residents earning income above the tax-free threshold (AUD 18,200 as published by ATO for FY25-26). The levy appears on your annual tax return and is used to provide free or subsidised care at public hospitals, GP visits, and specialist treatment.
Who Must Pay Medicare Levy?
All Australian residents earning assessable income above the tax-free threshold must pay Medicare Levy. Temporary visa holders (working holiday, skilled migration) are also required to pay while working in Australia. Some groups are exempt—see below.
Medicare Levy Exemptions
You may be exempt if you: receive certain government benefits (Age Pension, Disability Support Pension); are receiving treatment for a terminal illness; are under 21 and have no income; or are dependent children of exempt parents. International students with restricted work rights on student visas are sometimes exempt—check with the ATO based on your visa.
Medicare Levy Surcharge
The Medicare Levy Surcharge (MLS) is an additional penalty for high-income earners (singles earning above AUD 180,000; families above AUD 360,000 as published by ATO for FY25-26) who don’t hold private hospital insurance. The surcharge is 1–1.5% of taxable income above these thresholds, depending on the gap in coverage.
How to Avoid Medicare Levy Surcharge
To avoid MLS, maintain continuous private hospital cover rated as “Hospital” by the Department of Health. “Extras” cover (dental, physio, optical) doesn’t count. Join a private health insurer and maintain membership. If you exceed the income threshold, take out hospital cover before the end of the financial year to minimize surcharge liability for that year.
Medicare Levy Variation
If your income drops unexpectedly, you can apply for a Medicare Levy variation—paying a lower amount based on your current income rather than the previous year’s tax return. Contact the ATO if circumstances change (job loss, study leave, parental leave).
FAQ
Q: Can I claim Medicare Levy as a deduction? A: No, the levy is not tax-deductible. It’s separate from income tax.
Q: Do visa holders pay Medicare Levy? A: Yes, working visa holders are taxed as residents and must pay the levy.
Q: What if I don’t have private health insurance and earn over the threshold? A: You’ll pay the MLS. Joining a fund retroactively reduces but doesn’t eliminate surcharge arrears.
Q: Is Medicare Levy the same as private insurance? A: No. Medicare Levy funds public healthcare. Private insurance is optional and separate.
